Incident Reporting

Report incidents linked to clubs and societies confidentially through uea(su)’s process.

The health, safety and welfare of your members is incredibly important to us. If you experience any health and safety issues at your Sports Club or Society sessions that lead to an accident, incident or near miss occurring, please complete the below form as soon as possible. By doing this, we can support you to prevent future issues of this kind happening again and more importantly we can check in with you and your members to see that you are all okay.
